Since our school works with special needs students with IEPs, we as a staff are required to complete input and reevaluation forms for students. The feedback from the teachers regarding the students’ progress and classroom behavior is sent to the case managers to be updated to fulfill the IEP requirements. I was able to work with the special education teachers and counselors and create google forms that could be easily sent to the teachers and staff to complete and these forms could be returned quickly and effectively while staff is working remotely. 

       Below are some examples of the IEP Input forms. These forms can continue to be used once we are back in person.
